世界卫生组织(WHO)
(一)分类
妇科肿瘤(包括瘤样病变)的病理分类十分复杂，l近年则主要立足于组织发生、i形态及生物学特性方面的综合分类。
世界卫生组织(WHO)近5年内相继发表修改后妇科肿瘤的组织学分型。该分型立足于光镜、i电镜、i免疫组化及分子病理学基础上，l不断修改、i补充和更新，l已被国际上普遍采用。其中外阴、i阴道、i宫颈，l宫体、i卵巢的病理分类被采用于本书，l卵管、i韧带，l胎盘的病理分类则综合有关资料选写。现分别介绍如下：i

附11宫体肿瘤和瘤样病变组织学分类
(1)任何增生可能伴有局灶或弥漫分泌和(或)化生或有关改变。
(2)混合性癌系由子宫內膜样腺癌与其5种变型中任何1型或1型以上组成，l每型至少占肿瘤总体积的10％，l理论上，l应决定于子宫摘除标本，l若仅较小的标本时。第二种肿瘤亦需有足够成分方能诊断.应注明主要和次要肿瘤类型的比例。
(3)所有癌的腺体成分均采用GOG改良FIG0分级系统：i
Ⅰ级11非鳞状上皮性非桑椹体性实性生长方式占5％或少于5％。
Ⅱ级11实性生长方式介于5％～50％。
Ⅲ级11实性生长方式超过50％。
应注意Ⅰ或Ⅱ级的枝异型，l可以逐级上升。在透明细胞腺癌应首先进行核分级。
(4)在子宫內膜样腺癌中的一个实性巢应考虑戴性，l除非具备下列诊断标准之一，l方为鳞状上皮分化：i
1)呈示标准角化(以苏木精和伊红染色证实砧。
2)细胞间桥。
3)下列4种诊断标准中占3项或3項以上：i①片状生长不形成腺体或平行排列；②细胞边蠓蓑利；③胞浆嗜酸性，l增厚或毛玻璃样；④比同一种痔中其他癌灶的核浆比例减少。
(5)子宫內膜样腺癌申鳞状上皮实性巢被钮蚜学确定为恶性(腺鳞癌)，l反之，l组织学确定为良性(腺瘤伴有鳞状上皮化生)应符合下列诊断标准之一种或一种以上：i
1)标准的细胞学恶性。

在腺鳞癌中的鳞状上皮成分不需要分级。假若单独伎用腺癌伴有鳞状上皮分化，l則仅腺体成分需分级。
(6)未分化癌包括(但不限于)巨细胞、i梭形细胞及小细胞等变异，l后者可能舍有多量嗜银细胞及应与其他类型含少或多量嗜银细胞的癌鉴别，l后者仍诊断原来类型，l而独立注明其存在的嗜银细胞。现今嗜银细胞癌的命名尚未被接受。
(7)其他类型癌，l其更常发生于其他部分，l现报道发生于子宫內膜，l毛玻璃状细胞癌被列为腺鳞癌亚型，l疣状癌列为鳞状细胞癌型，l黏波表皮样癌柱列为黏液性腺癌亚型。
(8)任何子宫內膜腺癌可展示局灶或乃至广泛乳头状结构，l上述类型可称子宫內膜样腺癌内。“绒毛腺管”，l应着重其结构类型，l不能单独肯定为癌的类型(例如，l子宫内膜样，l浆液，l透明细胞)。
(9)间质性结节(肿瘤推向边缘)及低恶度子宫內膜间质性肉瘤(肿瘤浸润边缘)，l两者可能含有腺体。小管或性索样生长，l但并不改变其诊断。
(10)高恶度间质肉瘤现今尚不明其病因，l其可能有些、i大多数，l乃至全部肿瘤最终不能证实属于宫內膜间质型。
(11)富于细胞平滑肌瘤被限于为二个平滑肌瘤比其周围肌层有更多的细胞。
(12)各种上皮样平滑肌瘤包括平滑肌母细胞癌、i丛状型平滑肌瘤及透明细胞平滑肌瘤。
(13)退行变性.包括出血、i坏死及透明变性。可发生于任何平滑肌肿瘤，l不应为此更改诊断。
(14)诊断良性转移性平滑肌瘤，l可能仅见于回顾性(发生于转移后)。
(15)大多数形态和生长方式多样化“未肯定恶行潜在性平滑肌肿瘤”或平滑肌肿瘤，l诊断“未肯定恶性潜在性平滑肌肿瘤”的理由为（众多丝状分裂象但无非典型，l明显非典型及细胞众多但少量丝状分裂现象）。

(二)命名
1.命名原则11应能反映肿瘤的组织来源、i细胞瞰生物学行为等3方面的特点。
良性肿瘤11称××瘤。
恶性肿瘤。
上皮肿瘤11称××癌。
间叶组织肿瘤11称××肉瘤。
幼稚组织肿瘤11称××母细胞瘤。
不宜用癌、i肉瘤或母细胞瘤者11称恶性××瘤。
良、i恶性难分的肿瘤11称××瘤(附加交界性或潜在恶性)。
2.一般命名11肿瘤的组织来源+良性、i交界性、i恶性性质(相应结合组织形态特征)。例如卵巢浆液性乳头状囊線瘤、i卵巢交界性(低度恶性潜能)浆液性乳头状囊腺瘤、i卵巢浆液性乳头状囊腺癌、i卵巢宫内膜样间质肉瘤、i卵巢恶性腺纤维瘤(卵巢纤维腺瘤)、i外阴颗粒性肌母细胞瘤等。
3.特殊命名
(1)按肿瘤外貌特征命名：i如阴道葡萄簇状横纹肌肉瘤、i卵巢皮样囊肿、i卵巢巧克力囊肿、i卵巢胎儿形畸胎瘤等。
(2)按肿瘤细胞特征命名：i如宫颈毛玻璃状细胞癌、i子宫内膜纤毛细胞瘸、i卵巢宫内膜样癌、i子宫奇异性子滑肌瘤、i卵巢小细胞癌等。
(3)按肿瘤特殊排列方式命名：i如宫颈低离正腺癌、i宫颈腺样囊性癌、i子宫类似卵巢性索样瘤、i卵巢内胚窦瘤、i卵巢肝样癌、i卵巢环小管性索瘤、i卵巢多胚瘤等。
(4)按肿瘤内占优势组织命名：i如女性生殖器官中平滑肌、i横纹肌、i血管、i淋巴管瘤等。卵巢甲状腺肿、i卵巢皮脂腺瘤等。
(5)按肿瘤生长方式命名：i如外阴疣状癌、i外阴隆突性纤维肉瘤等。
(6)按肿瘤解剖部位命名，l如子宫静脉内平滑肌瘤、i胎盘部位滋养层细胞肿瘤、i卵巢门细胞瘤等。
(7)按肿瘤组成胚层命名：i如卵巢神经外胚瘤.同一胚层来源而有多种组织成分者称混合瘤。如子宫或卵巢恶性中胚叶混合瘤。不同胚层来源的多种肿瘤或组织成分组成。如卵巢恶性混合性生殖细胞瘤、i卵巢畸胎瘤。
(8)按肿瘤染色特征命名：i如女性生殖器官恶性黑色素瘤、i子宫及卵巢透明细胞癌、i卵巢嗜酸瘤细胞肿瘤等。
(9)按肿瘤组成成分命名：i如外阴血管纤维瘤、i宫颈黏液表皮样癌、i子宫平滑肌脂肪瘤、i卵巢性索间质瘤、i卵巢甲状腺肿类癌、i卵巢混合性生殖细胞性索间质瘤等。
(10)按肿瘤分化程度命名：i如卵巢胚胎癌、i卵巢成熟或未成熟实性畸胎瘤等。
(11)按肿瘤传统命名：i如卵巢无性细胞瘤。
(12)按首次报道人名命名：i如外阴柏哲(Paget)病、i外阴鲍温(Bowen)病、i卵巢勃勒纳(Brenner)瘤、i卵巢克鲁肯伯格(Krukenberg)瘤等。
4.瘤样病变命名11瘤样病变的定义是指所有类似肿瘤，l然而并不具备真性肿瘤基本特征的病变，l其可依据肉眼及(或)镜下检查证实因各种原因引起的肿块，l伴或不伴有细胞增生者。在妇科领域内涉及先天性者有女性生殖器官的中肾管或副中肾管残余囊肿(卵巢冠囊肿)、i卵巢表面上皮包涵囊肿及卵巢网囊肿等；涉及潴留性者有前庭大腺囊肿、i卵巢独立性滤泡囊肿或多发性滤泡囊肿及黄体囊肿等；涉及创伤性者有外阴或阴道表皮样囊肿；涉及局部组织增生者有外阴皮赘、i尿道肉阜、i宫颈及宫内膜息肉；涉及异位组织者有外阴的异位乳腺、i阴道的异位甲状腺或甲状旁腺及盆腔等处的子宫内膜异位等；涉及病毒感染及炎性病变者有外阴、i阴道或宫颈的尖锐湿疣、i卵巢黄色肉芽肿及软斑等；涉及类固醇激素影响者有卵巢多发性黄素化滤泡囊肿(过度反应性黄体)、i妊娠或产后巨大孤立性黄素化滤泡囊肿、i妊娠黄体瘤、i卵巢间质增生和间质性卵泡膜增生症等；涉及循环障碍者有卵巢巨块水肿；涉及局部细胞瘤样增生者有宫颈微腺体增生、i宫颈储备细胞及基底细胞增生、i阴道腺病、i子宫体间质性结节、i卵巢纤维增生症及卵巢门细胞增生等。
